Patient Care Manager
Responsible for the coordination, implementation and supervision of hospice services delivered by the interdisciplinary team. Requires RN license and 3 yrs mgt. experience.
RN Case Manager
Provides education, symptom management, and emotional support to patients. Strong knowledge of pain and symptom management required. Hospice or home care experience highly preferred.
Team Coordinator
Provides clerical support services to ensure quality patient care. Receives incoming calls, prepares correspondence, enters patient data, liaison between clinical and support staff. Requires: 3 years clerical experience preferably in healthcare setting, strong attention to detail, excellent organization and communication skills
Home Health Aide
Home Health Aide – Full Time
Provides personal care services according to the care plan. Requires CNA; hospice or home care experience preferred.
Social Worker
Provides initial psychosocial evaluations, counseling, and education to hospice patients and families. Assumes the role of advocate for the patient/family unit. Requires: BSW and IA licensure (or ability to obtain within 6 months) or MSW and current IA license. 3 years of healthcare experience preferred.
Chaplain
Provides spiritual care and support to patients/families. Master of Divinity and healthcare experience highly preferred.
Volunteer Coordinator
Attracts, recruits, and oversees volunteers. Works closely as a member of the interdisciplinary team to identify patient needs, manages assignments, and proper documentation. Volunteer management experience preferred.
Music Therapist
Plans, organizes, and directs music therapy services to address the physical, psychological, cognitive and social needs of patients. Requires: Graduate of approved college music therapy program. MT-BC designation (or eligibility to obtain within 6 months.)
